Signed application form with the following:

1. Copy of NRIC or Copy of Passport and evidence of Permanent Resident status (for Foreigner) and
2. Copy of Credit Card ( principal card , front side only ) issued by a domestic issuer and with a vintage of minimum 12 months
   
   

1. Copy of NRIC or Copy of Passport and evidence of Permanent Resident status (for foreigner) and
2. a) Copy of current or previous month's Pay Slip/Copy of latest Bank Account Statement where salary is
       credited, or
   b) Copy of latest Form BE with tax receipt or equivalent proof of income/EPF Statement, or
   c) Copy of appointment letter (if employed less than 3 months).

2. a) Copy of latest 3 months current or previous month's Pay Slip/Copy of latest Bank Statement Account where
       salary is credited, or
   b) Copy of Latest Form BE with tax receipt or equivalent proof of income/EPF Statement or Copy of Yearly
       Commission Statement from Employer.
   
   

1. Copy of NRIC or Copy of Passport and evidence of Permanent Resident status (for foreigner), and
2. Copy of Latest Form BE with tax receipt or equivalent proof of income/EPF Statement/Copy of Audited Financial Statement/Profit & Loss Account, and
3. Copy of Business Registration Form.
